This checklist will help interested hospitals prepare for enrollment in ÎÚÑ»´«Ã½NSQIP Pediatric. Following these guidelines will facilitate a streamlined process and understanding of ÎÚÑ»´«Ã½NSQIP Pediatric participation requirements.

Review ÎÚÑ»´«Ã½NSQIP Pediatric Program Resources
You will receive access to your center’s Qport following application submission. Log in and
review the ÎÚÑ»´«Ã½NSQIP Pediatric Resources.
Follow your internal hospital procedure(s) for participation in ÎÚÑ»´«Ã½NSQIP Pediatric
Helpful Hints:
- Obtain budget approval.
- Determine where to acquire CPT Codes of procedures. Case inclusion criteria must be determined by CPT Codes.
- Medical records must be documented in English.
Appoint your Surgeon Champion (SC)
- The Surgeon Champion provides leadership and supports the facility’s participation in ÎÚÑ»´«Ã½NSQIP Pediatric to achieve the highest-quality surgical care.
Obtain approval to hire a Surgical Clinical Reviewer (SCR)
- To achieve the highest-quality data, the SCR is required to go through training prior to data collection. We recommend waiting to hire the SCR until after the ÎÚÑ»´«Ã½NSQIP Participation Agreement is executed.
- Once we receive your application, the ÎÚÑ»´«Ã½Children’s Surgery Team will contact you regarding your center’s ÎÚÑ»´«Ã½Children’s Surgery Verification Quality Improvement Program Participation Agreement and annual participation fee.
